Chief Complaint:        
The patient reports a persistent sensation of something lodged deep within the throat. They describe it as “a word that refuses to leave.”

Physical Examination:
No foreign body observed. Airway patent. Mild Inflammation present despite the absence of infection.

Physician’s Notes:
The patient frequently hesitates before speaking, then stops entirely. Throughout the examination, the patient’s eyes remained fixed on the muted television.

When asked if the discomfort is painful, the patient replied, “Only when I stay quiet.”

DATE: February 25

Chief Complaint:
Persistent throat irritation. Episodes are most severe after returning from field assignments. The patient reports a sensation of roots tightening whenever they hesitate to disclose information they have witnessed following repeated warnings related to an investigation about the university. 

Physical Examination:
Inflammation persists. Fine, bark-like lines are visible inside the patient’s throat. Small green leaves have begun to emerge. No known medical condition explains the finding.

Physician’s Notes:
The patient repeatedly attempted to mutter sentences but failed to complete them. Coughing episodes occurred immediately after interrupted speech.

Chief Complaint:
Difficulty swallowing. The patient reports hearing “creaking” in the throat after withholding information.

Physical Examination:
The root-like growth has hardened into bark. Small leaves are now visible. The growth appears larger than it did during the previous consultation.

Physician’s Notes:
The pattern of growth cannot be explained by any known illness. When asked if the pain had changed, the patient said, “It grows whenever I leave something unsaid.”

Chief Complaint:
Speaking has become increasingly difficult. The patient reports a feeling of pressure, seemingly an internal intimidation, whenever they try to open their mouth. Episodes are becoming more frequent than during the previous month.

Physical Examination:
The growth has become larger. Thin branches are now visible. The patient pauses several times while attempting to answer routine questions.

Physician’s Notes:
Medication has produced no noticeable improvement. The patient attempted to describe a recent visit to a nearby community affected by a data center but stopped midway through the account.

No further explanation was given.


DATE: May 25

Chief Complaint:
Coughing episodes have become more frequent and are occasionally accompanied by leaves. The patient states that the condition worsens after being warned that certain information could no longer be safely published.

Physical Examination:
Wood-like tissue occupies most of the throat. Small blossoms were observed during examination despite the absence of any known cause.

Physician’s Notes:
The patient chose to communicate through writing. One sentence was recovered from the notebook:

“Silence was never my choice.”

This is the first examination in which a pattern becomes evident. The condition does not worsen after the patient speaks. It worsens after the patient is silenced.

A fully matured tree grows in the pharynx and extends into the upper chest. Blossoms remain healthy. The patient’s pulse is weak, yet the tree continues to thrive.

Physician’s Notes:
The patient attempted to say something several times. No words were produced. Only petals were observed.

FINAL DIAGNOSIS:

No existing medical condition explains the patient’s progression.
The tree did not grow from a disease.
It started from stories that were censored and forced to remain unpublished.
It branched out when students publishing them were harassed and threatened.

It grew from truths that had no safe place to be spoken.

There is no medicine for a voice that has been forced to be silent.
